Primary Care Network

Primary Care Networks (PCNs) are groups of GP practices working closely together - along with other healthcare staff and organisations - providing integrated services to the local population.

Since April 2019, individual GP practices have been establishing and joining PCNs covering populations of between 30,000 to 50,000 (with some flexibility).

The Lower Valley Primary Care Network (LV PCN) is an organisation made up of professionals and clinicians with the aim of providing care and improving the health of patients in the Lower Valley.

The Lower Valley PCN is made up of 5 GP surgeries:

  • Longroyde Surgery
  • Church Lane Surgery
  • Rydings Hall Surgery
  • Rastrick Health Centre
  • Northolme Practice

Any patients, of the 5 surgeries above, can use or access the services of the Lower Valley PCN team.

If you have an appointment with one of the services, the Lower Valley PCN building is located at the bottom of Church Lane, opposite side to Rydings Hall Surgery & behind Brighouse Pharmacy / Church Lane Surgery; first building in row.

How to enter the building - Press the buzzer outside which is located to the left of the entrance door.

On entering turn to the right and, if a receptionist is present, report your arrival. Otherwise take a seat in the waiting area and a clinician will be with you shortly.

Parking

Patient parking is limited to three spaces in front of the building. These are prioritised for blue badge holders and people with disabilities.

All other spaces are leased and cars parking in them may be clamped.

Free parking for up to 1 hour is available in the Calderdale Council car park on Church Lane (HD6 1DL) .
